Credit memo sample (filled in)

This is the same layout with every field completed, so you can compare your own credit memo against it. The names and figures are made up. The sample credits one returned item and one agreed price reduction, and it has no sales tax line because no tax is being credited.

This is the billing credit memo a seller issues against an invoice. It is not a bank loan document, and this site does not provide loan paperwork. A credit memo line on a bank statement is something else again: see credit memos on a bank statement.

Credit memo format: every field, in order

The document is headed CREDIT MEMO and lays out top to bottom like this:

  1. Credit memo no., Date issued and Original invoice no., the field that ties the credit to the invoice it reduces.
  2. Reason for credit: return, damaged goods, pricing error, cancelled item or an agreed reduction.
  3. From (seller) and Credit to (customer), with lines for name, address and contact details.
  4. A table with DESCRIPTION, QTY, UNIT PRICE and CREDIT. Blank rows: 6 in the PDF, 7 in the Word file and 10 formula rows in Excel.
  5. Subtotal, Sales tax credited and TOTAL CREDIT, right-aligned under the table.
  6. Notes (PDF and Word): for example how the credit will be settled.
  7. A printed line at the bottom: "A credit memo reduces the amount the customer owes on the referenced invoice. Keep it with the original invoice."

What the Excel version calculates

You type into the outlined cells. CREDIT on each row is quantity × unit price, Subtotal sums the credit column, Sales tax credited is the subtotal times the rate you type into the Sales tax rate % (edit) cell (rounded to two decimals), and TOTAL CREDIT adds the two. The rate cell starts at 0, so no tax is added unless you enter the rate you charged on the original invoice.

How to fill in a credit memo

  1. Give it a number and a date. Use your own credit memo series (the sample uses CM-1001) and the date you issue it.
  2. Name the original invoice. Write the invoice number in Original invoice no. so anyone checking the account can match the two documents.
  3. State the reason. One line is enough: "2 units returned damaged", "price agreed at $15 less per unit".
  4. Fill in both parties exactly as they appear on the original invoice.
  5. List what you are crediting: the returned items or the reduction, with quantity and unit price.
  6. Add the sales tax you are giving back, if the original invoice charged tax on those items. In Excel, type that rate into the rate cell; on the PDF or Word file, write the amount on the Sales tax credited line. Otherwise leave it empty.

Credit memo or credit note?

Both names describe a document the seller issues to reduce what the customer owes on an invoice. This template uses US wording ("credit memo", "sales tax credited"), with the PDF and Word files on US Letter; our credit note template uses international wording ("credit note", "tax reversal") on A4, and our UK credit note template adds the VAT fields HMRC lists. Is this credit memo template free to use?

Yes. The PDF, Word and Excel files are free to download, with no signup and no email. Each file carries one small line that points to our online generator (at the foot of the PDF and Word files, under the title in Excel); in the Word and Excel files you can delete it.

Is the PDF credit memo form fillable?

No. The PDF is a printable form without form fields, so you print it and write on it. To type into the form, download the Word file (type into the fields and the table) or the Excel file (the totals calculate for you).

Can I edit the credit memo in Word or Excel?

Yes. The Word file is an ordinary, unlocked document: change the wording, add your logo and save it as your own credit memo. The Excel file has input cells for the header, the parties and ten credit lines; the CREDIT column, Subtotal, Sales tax credited and TOTAL CREDIT are formulas. Both files were opened and checked in Microsoft Word and Excel.

What paper size is the credit memo template?

The PDF and the Word file are US Letter (8.5 x 11 in). The Excel file has no paper size saved in it, so choose Letter under Page Layout in Excel before you print it. Our other credit document, the credit note template, is on A4.

Can I make a credit memo online instead?

Yes, with the Credit Note Generator. Type CREDIT MEMO into the Document Title field and the heading of the PDF changes; the rest of the PDF keeps its labels ("Credit Note No.", "Credit To", "TOTAL CREDIT"). The tax row starts as "Tax" at 0%, so type your sales tax label and rate if you are crediting tax, and pick Letter under Document Format if you want US Letter paper. The generator is free and unmetered for normal use; an anti-abuse cap of 20 PDFs per device per day (200 per IP) applies, well above ordinary use, and the blank template downloads on this page are not metered. A one-line "Created free with myinvoicetemplate.com" credit is switched on by default and one free checkbox in the settings turns it off. Your draft is stored in your own browser on this device, so download and keep the PDF you need.
